We are seeking participants with type 2 diabetes who use insulin and whose diabetes has affected kidney function and are interested in trying out a new medication that is expected to improve blood sugar control and prevent the weight gain (or induce weight loss) associated with increased insulin use. This is a clinical research study opportunity for people with Type 2 Diabetes using long-acting insulin. In this study, we want to learn more about an innovative approach for improving diabetes outcomes using Fractyl Health's Revita System for removing excess tissue from the small intestine. This minimally invasive procedure is believed to improve blood sugar control and overall metabolic health by altering the body's hormonal response to food intake in a similar way to that achieved with bypass surgery. The purpose of this study is to compare diet (record of the foods you eat), gut microbiome (stool sample), and cells (blood sample) in adults with IgA Nephropathy (IgAN) and compare with adults with focal segmental glomerulosclerosis (FSGS) and adults with no history of medical problems. The information we learn by doing this study may help us understand how we might be able to target the microbiome to better understand the disease process and potentially help us develop new therapies in the future.
